Ehlers-Danlos syndrome (EDS) is a heritable group of disorders of connective tissue characterised by skin hyperlaxity, joint hypermobility and tissue fragility. We report on a 49-year-old man with EDS who presented with spontaneous swelling at the root of the right neck during coughing. This soft expansile mass is actually due to increased protrusion of the lung through the superior thoracic aperture, and is regarded as pulmonary apical herniation. In our experience supraclavicular herniation of the lung is not unusual.

We report a rare case of a patient who was recently encountered in our Otorhinolaryngological Clinic and who posed an interesting problem of differential diagnosis.

In our experience supraclavicular herniation of the lung is not unusual. Apical 'herniation' of the lung is an unusual protrusion of the lung and its pleural coverings through the superior aperture of the thorax. It is supposedly caused by weakness of Sibson's fascia. Apical lung hernia is a rare variety and has been confined to few case reports and series. Herniation occurs through a defect in the Sibson's fascia and the apical segment of the lung protrudes in between the scalenus anterior and sternocleidomastoid muscles. This lung herniation was probably caused by a congenital deficiency in the suprapleural membrane (Sibson’s fascia), combined with increased thoracic pressure created by the respiratory tract infection. Surgical repair was not necessary as the hernias were asymptomatic and not associated with chronic cough.
